REQUIRED DOCUMENTS (CNA/HHA)

  • Social Security Card
  • Alien Registration, Work Auth. or Proof of U.S. Citizenship (Voters Card/Passport)
  • CPR Card
  • Active Auto Insurance
  • CNA License or HHA Certificate
  • Level II FDLE Background Screening (You Cannot Work Without This!)
  • HIV/AIDS Original 4hr certificate and HIV/AIDS update
  • OSHA 2hr Certificate
  • Domestic Violence 2hr Certificate
  • Alzheimer’s 4hr Certificate
  • Assist with Medication 2hr Certificate
  • Professional Liability Insurance (see office for application)
  • Physical within the Last 6 Months Signed by Medical Doctor, ARNP, P.A. or D.O. Stating “Free of Communicable Disease.”
  • Voided Check for Direct Deposit
